The Client Portal is a link for new and established clients to access an encrypted, secure scheduling and record system.
If you are interested in becoming a patient at SOAR, you may contact Dr. Martinez (214-810-4667; [email protected]) to set up a brief phone screening in which we will determine if you are an appropriate fit for the services offered. If we decide to set up an intake assessment, I will email you informed consent and privacy practice documents to sign as well as an intake form to complete through my secure, encrypted electronic medical record system. Alternatively, you may follow the link to the Client Portal, select a date and time to request an appointment, and enter your name and contact information. (Please note: if the date you would like to be seen is within 48 hours, you will be prompted to call to schedule.) I will get back to you as soon as possible to confirm your appointment date and time and will share intake paperwork that should be completed prior to our first visit.
If you are currently a patient of Dr. Martinez, you may access the Client Portal to request an appointment, access or change the intake documentation you shared, see a detailed listing of your invoices and payments, and access insurance reimbursement statements if you choose to file for out-of-network benefits. Your login information is your email address and the password you created after logging into the system for the first time.
